Client Forms

A link can be emailed to clients to create a client portal through our secure site, Simple Practice. All forms can be reviewed, signed, and returned through the client portal. All forms are requested to be signed and returned before the first session. 

If you feel that you would like to collaborate your session with another provider, simply request an ROI form that can be filled out and returned through the client portal.
